You get your medical doctorate (MD) when you graduate medical school. That is not something that can be taken away.
You get board certified after passing your specialty/subspecialty board exam. You usually have to retake your board exam every 10 years. Board exams cost thousands of dollars just to sit for.
You get licensed with the state in which you work after jumping through many bearucratic hoops. You typically have to renew your license every 1-2 years, which costs hundreds to thousands of dollars each time. After reading about Dr Been, it sounds like he practiced medicine (therefore was licensed and board certified) for a few years before transitioning to what he is now. If he’s not practicing medicine, it would be pointless and financially irresponsible to stay board certified and licensed. By no means does that means he is not a doctor and does not have an MD.
Having said all this, I’m not sure why it matters to you.
