I would opt for no stent at all since I take the pills that help prevent me from needing a stent. However, were I a non pill taker, suspicious of the evil pills that kill people (the internet tells me so) then I would opt for BMS if my vessel diameter was 3mm or greater or DES if smaller - or diabetic, since I am of average bleeding risk. If I had surgery coming up or knew I was a bleeding type of person I would roll the dice with BMS.
The late thrombosis risk of DES is so much lower than the restenosis risk of BMS that it makes me favor taking the plavix pill rather than dealing with restenosis, which is a bad, bad road to go down, believe me. Radiation is no longer a valid option and it sucks, anyway. You have to see these scenerios play out in real life with loved ones before you get an appreciation for how hard these decisions become to real people and what the stakes are.
OK, let's say you opt for BMS and get restenosis.... Now you get a DES inside of your sucky BMS and guess what? YOU NOW HAVE THE DREADED LATE THROMBOSIS RISK THAT YOU SO DESPERATELY TRIED TO AVOID IN THE FIRST PLACE! Join the club of people who would rather be instumented multiple times rather than take evil cholesterol pills (THAT KILL PEOPLE ACCORDING TO THE INTERNET! the holy frickin grail of knowledge).
Take your poison early or take worse poison late. I'll choose combination lipid therapy any day and pony up at the pharmacy and contemplate how my grandchildren can be genetically altered to not have to face the decisions I do.
For the record and bear my words.... I will never, ever get a stent of any sort since I stake my life on proper, and very aggressive, medical management of my risk factors such that I have a NORMAL <120 systolic BP, a LDL cholesterol particle number less than 1000 (by NMR), a small LDL particle number less than 600, a HS-CRP <1 and a Lp(a) < 30 and a HDL >50 and having watched family members live and die with gut wrenching device decisions I hope I never have to go there.
To the Docs on this board I challenge you to offer all interested patients the same aggressive management (whether they take the advice or not). I would also ask the Docs on this board whether they treat their patients as aggressively as they treat themselves or their family members? If you don't then SHAME ON YOU!!! READ YOUR HYPOCRATIC OATH or remain a hypocrite.
I stumbled into a doctor's lounge after my brother coded and died and listened to one of them brag about how low they had their LDL with drug company supplied free pills and I almost evened the score. It makes my blood boil just to think about it.
