You may be correct. At what level the decisions are made probably depends on each individual institution. I’ve seen several situations at institutions where high level administrators made decisions without clinician input and even some where it was known the clinicians weren’t on board with the decisions, and the results were sometimes really bad in terms of retaining functional staff and in terms of patient outcomes. Both of these had further widespread blowback including one where the state government intervened to audit the programs. Yikes! None of the clinicians were surprised at this, but the disconnected administrators were.
If they do create something that is truly an improvement, then it will become implemented eventually. But that road will undoubtedly have bumps, setbacks, and resistance along the way. I guess we’ll see what happens.
