I have wondered about that too. Could one explanation be that it is rather rare for 2 biologics[D + B] owned by 2 different companies, neither of which had approval for any indication, to be used in combination therapy? What FDA permissions, if any, would be necessary for that combination therapy to occur?

Now that Durvalumab has been approved by FDA for one indication this past Monday, is it reasonable to expect a status change?:
