in trials, to have the originator keep full quality control, he would send out finished bulk, for the vial fill stage
and
in commercial usage, the hospital pharmacy never uses oncology type bulk but obtains batches of vials, and dispenses to the infusion room, on a day-specific prescription by the Oncologist, a vial(s) bled to the correct volume that the (weighted) patient requires for infusion at that time.
Which is why of all places in the USA, the metric system is being used in hospitals, and even Emperor Trump will not change this ;-)
