Remarks DURING EMER EVACUATION DRILL, THE DOOR 5R FAILED TO ENGAGE INTO THE ARMED POSITION, THUS PREVENTING THE DOOR SLIDE & RAFT TO DEPLOY PROPERLY. OPERATIONAL & INSPECTION CHECK REVEALED THAT ALTHOUGH THE ARMING HANDLE TRAVELED FROM THE MANUAL WINDOW TO THE AUTOMATIC WINDOW THE FULL TRAVEL OF THE HANDLE WAS NOT REACHED, APPROX 1/2 TO 1 INCH SHORT. THIS WAS DUE TO THE BAR LOCK ASSEMBLY BUTTING UP AGAINST THE UPPER TAB OF THE FLOOR BRACKETS & NOT SLIDING UNDERNEATH & INTO THE BRACKET AS REQUIRED. FURTHER ENTRY DOOR LOWER GATE ROD TO LIFTER ASSEMBLY ADJUSTMENT WAS NEEDED TO FULLY SEAT THE BAR LOCK ENGAGING ROLLER ARM INTO THE BAR LOCK ASSEMBLY FOR SLIDING INTO THE FLOOR BRACKET.

IT WAS DISCOVERED THAT THE GIRT LANYARD HAD TORN AWAY FROM THE STITCHING ON THE GIRT APRON SOMETIME DURING THE INITIAL DEPLOYMENT SEQUENCE. THE FAILURE OF THE RELEASE CABLE & PINS MECHANISM TO FULLY RELEASE MAY HAVE PUT ADDED TENSION ON THE LANYARD STRAP CAUSING THE RELEASE LANYARD STRAP TO TEAR AWAY FROM THE GIRT APRON THUS PREVENTING ANY CHANCE OF THE SLIDE & RAFT TO RELEASE DURING THE REMAINING DEPLOYMENT, INFLATION, SEQUENCES. A FOLLOW UP CONTROLLED PULL, USING AN ELECTRONIC SCALE, OF THE REMOVED SLIDE & RAFT ASSEMBLY GIRT LANYARD STRAP WAS ACCOMPLISHED TO ASCERTAIN THE PULL FORCE REQUIRED TO RELEASE THE PACK BOARD CABLE & PINS MECHANISM FREEING THE SLIDE & RAFT. A PULL FORCE OF 128 LBS WAS RECORDED ON THE SCALE. FURTHER INVESTIGATION OF THE GIRT LANYARD STRAP & THE GIRT APRON REVEALED AN OVERALL AGED CONDITION.

WE HAVE SENT THE UNDEPLOYED SLIDE/RAFT PACK BACK TO THE MFG O/H FACILITY FOR FURTHER DETERMINATION OF THE FAILURE ON THAT PARTICULAR ASSY. WE WILL ALSO CHECK, RE-RIG AND LUBRICATE THE DOOR/POWER ASSIST IAW THE AMM 52-11
