As your link states, fails can happen for several reasons...in this instance, 10 shares failed for 5 days for whatever reason--and looking at the historical were "covered" as soon as there was any liquidity---this issue only traded on half the days in question during that reporting period...
and zero FTD's for the previous two months---I didn't look back any further since I fail to see an issue here...
if there is any" manipulation" in this most thinly traded stock, you can count on it being whomever the MM is routing for...
