I still don't see how you can get a 60% expansion in throughout without some help from PCPs. Even if the PCPs limit their involvement to screening and general awareness counseling (for HCV patients and family members), they can ease the bottleneck to a material degree.

Had my views changed about which specialties will treat HCV, after talking with an ID doc. Major bifurcation between integrated systems - Kaiser, Va, etc. - and private health care. In integrated systems, ID docs are arguing for a much larger role because they are the ones with all of the HIV experience around similar drugs, and since resistance is still an issue, the ID docs feel they can better manage that. Besides in integrated systems, the issue of which specialist can bill for what procedure/diagnosis is not an issue.
