I use BSBC PPO. The insurance company decides, based on their Tier system, what they pay for each drug.

Before I had HMO when a physician had to justify prescribing high priced drugs. My physician did not like to do it and I had to use high power specialists to get drugs I wanted.

Making a long story short, insurance companies have plenty of leverage.
