I spoke with the Coffee Hub and Metro Pizza/Tropicana in Vegas, as well as Piccolo Mondo who is shown on the Epoxy map as being in Florida, but actually does business in Texas.
Left messages with a candy store in Vegas and a salon in the Midwest. None of these companies are national.

Both Metro Pizza and Coffee Hub are reported the app is highly used. Metro said a lot of people will come in and use the app in waves, lots of use all of a sudden followed by none and all...rinse, repeat.
I received an emphatic YES when I asked the Coffee Hub if lots of people were using the app.
Both of these places said people seemed to really like the app.

Piccolo Mondo has never heard of the app and was very surprised to learn I found them on it. A very nice woman told me there is some sort of database that apps like this can pull info from. The restaurants have no say in the matter. This one could have been a test? My Piccolo contact downloaded the app but didn't see her restaurant on the map...strange.

It sounds like NHYT is actively promoting it..the waves of epoxy users at Metro hints at reoccurring awareness surges, and in sure their billboard doesn't hurt either :). I wonder how else they are advertising....
