I hear ya and that was a legitimate concern that was raised by a number of folks via PM, however, ultimately all you are submitting is a letter from your broker stating your name and your holdings, that is it, any other personal info or holdings that may be on there can be blacked out if you so choose.
I don't know exactly what happened but according to my broker the depository that holds their shares was unable to complete the exchange with the company's TA. They said that their notes indicated the company either canceled or was unable to deliver the new RAFA shares and that the depository was no longer expecting to receive the new issue.
So the bottom-line is that if you do not follow this procedure with submitting proof of holdings to to company to pass on to the shareholder who is giving us the shares you have zero chance of getting them.
