How realistic is it for this to ever reach the market given GILD's in-house strategy?
I certainly don't think GILD will move the combo forward given, as you note, their penchant for wanting an in-house combo (can't blame them). For this to be an option for JNJ/Medivir, they would presumably need the GILD in-house combo to fail, or at least disappoint, in the clinic.
Or do you think it'll get used "off-label" when both launch in combo with IFN?
I honestly don't know here. I've seen on answers on both sides here and I have no idea if off-label use will happen.
Either way, I think the simeprevir+GS-7977 is very important for Medivir because it gives us the first evidence of how well their drug plays with other HCV DAAs. I think that's very important. We need some assurances there won't be safety issues for simeprevir when used in combo and it will good to see what kind of efficacy can be obtained when it's used as part of a combo, even if this specific combo may not see use commercially.
