"Single Payer" - after a high deductible - to be determined - but as high as 50k to start, imho. The only way to rein in costs, at this awful point, is for the Gov to take over and set prices on the big procedures, medical supplies and drugs. There is no way it can be done with the type of mixed up private / public system we have now or worse with Obamacare. Since the private insurance plans would be responsible for the up / front costs only, those would be affordable. The very rich may decided to forego and pay their own way during the deductible phase. Employers could easily afford to offer a variety of supplemental plans and the States would not have the unworkable burden they now face with Medicaid. Cost savings also from the feature that "Personal Responsibility" in one way or another would be required in the up/front phase during the deductible. As it is now many 'think' that someone else is paying for thier over use of care.....
