Another tough question, and one that is only alluded to in Scripture. I believe scripture teaches that young children and those who are mentally incapable of understanding or making the decision are covered and will be found in Heaven. That is a personal belief derived from a number of passages, and not the result of clear revelation, so it is no more than my opinion.
The question of all those who have never heard the gospel (and there have been millions) is even more difficult. The Bible does teach that all men have been exposed to evidence of God in the universe and all creation and have the opportunity to accept or reject that imperfect revelation. Exactly how God handles that is not told in the Bible, but we can rest that His method of handling the problem is perfectly just and merciful. My personal belief is that much like in the Old Testament times before Abraham, faith in God based on what you know is what is required for salvation. Most certainly it would never be based on good/bad deeds, since only one sin is all it takes to separate one from God eternally.
There are many questions that will only be answered when we are before God.
