so, who decides what you get?

how does Vermont pay a doctor in New Hampshire.....or, one in Florida (if your on vacation)???

does vermont set the doctors' fee schedule??

how about testing and diagnostics?? the state is the only payer.....in business if you are the only vendor in a market.....you control the pricing.....that's why corps fight to be #1......#1 is in control.....
