Actually, usually not. SPIN strongly recommends that the local clinic brings in a Board Certified Pain Specialist that usually has a first or second specialty as an anesthesiologist. There are quite a number of these nationwide that do not have their own practice per se, Ie. no bricks and mortar but "hire out" sorta like a Paladin, "Have Needle, Will Travel". This gives the local clinic another level before them of potential Medical Mal Practice insurance that the specialist must carry.
The average clinic when in session will usually do 20-40 procedures in a day. So even if the local clinic pays, say $200 a procedure to the specialist, the specialist will bring in $4-8k or even more for sticking maybe 40 needles in a day. Not bad since he really has no additional overhead. However, the local Orthopedic Surgeon will do the individual pre and post procedure consults with the patient so they do get a rapport.
