Yahoo is probably counting NVS as an “insider” because NVS used to be a 10% shareholder and 10% shareholders are subject to the same reporting requirements as insiders.

Moral: Do not take the numbers on Yahoo Finance as the gospel truth. When in doubt, check the SEC filings.
