I received something similar, except there's a big fat "Do Not Mail" over the right side. No checkboxes, signature lines or anything else on the right side, just blank.

There is a note above this stating that the ballot is for informational purposes only and I should contact my financial institution (Firstrade) if I wish to make an election.

I got the package w/ CD, no return envelope, though.

Anyone else get something like this? Do I need to vote by phone or do I not get to at all? Shares were purchased in Nov. 2008.
