No, you do not need 100% conjugation efficiency for the drug to be effective. In fact, I think that if you loaded up all 86 available lysines with drug, the antibody toxicity would be off the charts making the drug unusable You have also pointed out the other problem with 100% efficiency... the mab would most likely be unfolded or damaged during the process.
