I know you asked this question last week, and while I do not have any market information, I take a stab at an answer. In my experience as an internist taking care of medically ill patients and particularly as a consultant in general surgery and orthopedic patients, the majority use of Lovenox in an acute care setting is for deep venous thrombosis(DVT) prevention. The most likely dose is 40mg daily. The second most common dose would either be 30mg daily for DVT prevention in the elderly or some orthopedic surgeons like to use 30mg twice daily. Preventative treatment typically continues throughout the hospital stay, and if a post-op orthopedic patient, may continue at home, in a rehab facility or a skilled nursing facility for a total of 20-30 days although it varies widely per surgeon as there is no orthopedic standard (if you can believe that!).

Higher doses of enoxaparin are used to treat actual DVT or pulmonary embolism (PE). The dose is weight dependent and given at 1mg/kg every 12 hours. If you have a 175 pound person with a DVT or a PE, you would treat with 80mg every 12 hours. One would typically initiate warfarin shortly after diagnosis, so overall duration of treatment may only be 5-7 days on average.

I do not know the average weight of people developing either DVT or PE.

At our acute care hospitalof 400 beds, the breakdown in dosing is approximately:

40mg daily dose - 55%
30 mg daily or twice daily - 25%
80mg twice daily - 8%
90mg twice daily - 8%
other (higher) dosing twice daily 4%

In the rehab unit with an average stay of 11 days, dosing would be:

40mg daily - 70%
30mg daily - 20%
30mg twice daily - 5%
80-90-100mg twice daily 5%.

BTW, treating PE or DVT in pregnancy precludes the use of warfarin, so a woman would be on high dose Lovenox for the duration of pregnancy and not switch to warfarin. Also used for other thrombotic states of pregnancy.
