Bob, regarding the survey.

Take it easy. I'm sure you could have easily added a "skip" link on the survey. That information is valuable and should be collected.

Most surveys I come across ask you "Would you like to take a survey?" or something of that nature. Perhaps that style should be used.

I think a survey is a good idea though.
