it occurred to me that this will slow things down -

Have been in other programs that seem to have had the same troubles with forms and notification of participants, and the outcomes were not fortuitous to me. I am still going to be positive in this matter though

Again, there are all kinds of forms we fill out directly online, which information becomes instantly available on the other end.

maybe they can come up with something of that nature - or just input what they get, do a sort, eliminate the duplicates, and try to contact those who have not submitted.

Long ago we were requested to provide info -
Name, address, phone #, e-mail address, SSN/tax #, DOB, certificate # and # of shares - this was just at the end of 2008 - I neglected to date my fax

Cy of e-mail 2/17/2009 stating "for those of you who have not been called or have not sent info, you can fax toxxxx# or call one of the gals below xxxxxx (from my group communicator) It stated both SSN/govt ID and birthdate required.

So, I would presume this current item is just to verify against what they already have for communication with us.
