I think it is worse than that in that the insurance companies may now have incentives to get the "rate base" up by letting costs and hence margins rise faster. A public option/single payer probably doesn't help given fraud seen in Medicare and Medicaid and the ability in past for insurers to control cost increases to lower levels.
