There was an earlier post from Joyce Presnall (EC) stating that the site is legit and no information would be used by anyone except the EC. FYI, the site asks for name, address, phone no. , and email address and no. of shares of each stock: WAMUQ, WAMPQ and WAMKQ.

I'm VERY cautious about giving out personal info. but after Joyce said it is OK, I sent my form in. I also made a small donation for the cause.

If you're not comfortable, just don't do it.
