Is there any evidence yet of specific advantages/disadvantages, either in terms of efficacy or safety, to targeting the "thumb" versus "palm" site?

Not that I know of. The relevance of the distinction is that it may make sense for an HCV cocktail to include both a thumb binder and a palm binder, but it probably does not makes sense for a cocktail to include two thumb binders or two palm binders.
