To be honest, I don't really know if there are independent sales and marketing firms specializing in Pharma sales.
There are marketing firms for medical devices, so I would bet there are similar firms for drug sales.
If not, how do you go about marketing an orphan drug effectively? Is it simply missionary sales to those MD's specializing in that disease?
