Not sure. But my broker printed out all of my transactions from Aug. 05, with confirmation numbers AND cusip numbers, and I sent them in, along with my responses. Your broker should do the same for you. Just tell them you want copies printed out of all of your bcit confirmations. along with cusips. Laura told me to go ahead and block out account numbers. She said they DO NOT need personal financial information.
