You almost never see this done at any company unless the options in question are about to expire. Mathematically, it doesn’t make sense to do this unless the holder is close to 100% certain that he plans to sell one year later.

It’s the fact that insiders are not selling shares that is bullish. The lack of option exercises is merely a manifestation of the lack of interest in selling.
