I never received a card..one of my friends in Toronto did.
I called the fund and spoke with a woman.
I gave her my name and she was able to verify my claim had been submitted and she gave me a claim number.
She did not ask for any other details at that time and told me if more information was required she would be in touch.

I suggest you also make the call and verify your claim is active.
The cards they send out are small.
It's possible I received the card with other junk mail and it got tossed.
