Looks like its primiarily HexaFluorosilicic acid thats added to water. Source mainly from fertilizer industry. Pharmaclogical grade used in medicines & toothpaste is sodium fluoride; also used in some smaller municipal water systems. So definitely not Fluorine, its gaseous form. Fluoride seems unstable compared to Fluorine and likely looks to bind with other elements.

Is adding it to water necessary? People in rural areas do not ingest it. Do they have more dental issues than in city broken down by similar economic demographics?

Just because our bodies need trace natural elements does not mean we need greater quantities artificially added. That line of thinking would mean we need more Thorium in our diets, which if I'm recalling correctly is naturally radioactive.
