how is off-label use compensated?

Dunno. Since I have not heard of any patient getting the clotting factor in 20 yrs of primary care practice, my hunch is that most of the recombinant stuff is used in Europe where there is a confusing (to me) array of private, insured, and gov't subsidized health care. I suspect a significant amount of the drug is private pay by people who are wealthy.
