For I-O regimens, OS is clearly the most consequential endpoint, but companies typically want a faster-reading co-primary endpoint (usually PFS) to enable an Accelerated or accelerated (with a lower case ‘a’) approval. When using multiple co-primary endpoints of which one is OS, the OS endpoint usually gets a large share of the alpha spend.
