If you want proof that it happens you need look no further than the format of almost every single 10-Q or 10-K submitted to the SEC. They all have the same format because they all use a template.

It's not a crime, or fraud, or anything nefarious to start with a template when drawing up documents for your business, rather than start from scratch.
