Unsuccessful slide deployment Service Difficulty Report
Just checked and here's the narrative from the Baltia SDR on the FAA web site.
Occurrence date was 7/28/15 and reporting date was 8/4/15 (several days late).

R1 EVACUATION SYS PN 7A1238-64, SN G1974, FAILED TO DEPLOY SUCCESSFULLY DURING A SCHEDULED DEPLOYMENT TEST FROM ACFT REGISTRATION.

They have another one about an emergency exit light problem in June that was reported over a month after the occurrence. That doesn't seem to be associated with any exercise probably just the preparations.
