5 1/8% is the going rate as of Friday for a regular 30 year conforming fixed mortgage up to $322,700 loan amount. 15 year is usually a lower rate. Adjustable loans start even lower, and some hybrids (fixed first 5 yrs, then adjustable) are around 4.25%.
I really don't know what the refi limits are on cash out. In my case, the only limitation was the loan to appraised value ratio had to be less than 65% (independent of the existing loan amount) for a cash-out refi. I don't know if one simply pays more points or something like that for an LTV higher than 65% or if 65% is the max they will do.
