It's either the “sink hypothesis” is wrong or they are missing something so it is not complete. But in any case, I think the “sink hypothesis” has been discredited by the latest Solanezumab data.

Solanezumab binds soluble a-beta. According to the "sink hypothesis" removing peripheral, soluble a-beta should impact the insoluble a-beta deposits in brain plaques, in order to maintain equilibrium between the two forms.

I'd suggest it would depend on:

a) The relative amount of insoluble a-beta vs soluble (I'd expect there to be a lot higher density of insoluble - so the sink would be disproportionate)

b) The rate constants that effect the conversion from insoluble to soluble. Given that it is plaque, I'd expect it to be 'sticky' - i.e a low rate constant.

I don't know either of the above parameters, but my question would be how confident are people that we do know those parameters?
