HCV: Most Likely to Succeed (IMHO)

[Updated entries for IDX320 (graduated from paragraph 4 to
paragraph 3) and ACH-1625; new entry for unnamed 2nd-gen
PI from Roche/ITMN.]

The following paragraphs are in descending order of likelihood of success. There is no claim of completeness in this compilation, i.e. paragraphs 2-6 do not necessarily mention all of the applicable drug candidates within a given grouping.

Please see #msg-49115388 for the distinction between a nucleoside and a nucleotide; also see #msg-43114117 for historical perspective from the HIV arena.

1. The two leading protease inhibitors: Telaprevir (VRTX/JNJ; phase-3) and Boceprevir (MRK; phase-3). Between these two, Telaprevir must be considered a slight favorite because it has shown stellar efficacy in one phase-3 trial, while Boceprevir has yet to report any data from phase-3. Moreover, insofar as Boceprevir is being tested in a non-pivotal phase-3 trial focused on treating anemia, there is some question as to whether MRK will actually submit the Boceprevir NDA in 2010 (#msg-50187183, #msg-50261435); if MRK does not submit the NDA in 2010, Telaprevir will have a significant timing lead.

Telaprevir in the treatment-naïve setting: #msg-50595952 (results of phase-3 ADVANCE study—PR); #msg-50595752 (results of ADVANCE study—data table); #msg-50633310 (tepid investor reaction/Stevens-Johnson Syndrome); #msg-26228377 (ILLUMINATE—the smaller of the two phase-3 trials); #msg-43114192 (C208 study of BID vs TID dosing); #msg-29019931 (summary of results from phase-2b); #msg-28746843 (detailed results from phase-2b).

Telaprevir in the second-line setting:: #msg-32901932 (phase-3 REALIZE study); #msg-48759554 (phase-2b PROVE-3 study).

Boceprevir in the treatment-naïve setting: #msg-29474929 (overview of phase-3 program); #msg-50187183, #msg-50261435, #msg-37298085 (questions about anemia and EPO use); #msg-42086185 (AASLD 2009 abstract re phase-2b SPRINT-1 study); #msg-37298987 (interim SPRINT-1 results at EASL 2009); #msg-31190433 (Dew’s musings on SPRINT-1 data).

Boceprevir in the second-line setting:: #msg-29474929 (phase-3 RESPOND-2 study).
