Sts66, BCBS-FEP plans excluded V from formulary but cover GV on formulary. Some others, mostly government payers, that use closed formularies have done the same. Closed formularies restrict access to many brand and specialty meds and almost exclusively cover generic alternatives only on formulary. Our argument is that GV is not indicated for CVD so there isn’t a generic substitute or other trig lowering alternative that is indicated to treat CVD.
