In defense of doctors, there are over 19,000 FDA approved drugs on the market. I often wonder how they are expected to be familiar with the nuances of all of them. Granted, a lot of them treat ailments that are extremely rare, so doctors don't need to be familiar with all of them. But if they are "Nissen disciples" (and how would they know any better?), then they are following that data.
