You need to have diabetes or heart disease and then two more risk factors. I don't believe stress & family history qualify. If your triglycerides are 150 and above, you qualify for on-label.

I am a moderately healthy guy and wouldn’t qualify under this label, but can 2 risk factors be stress and family history or does or does it have to be more ‘documentable’ things?

Retire -Family history of early CVD (generally defined a CVD <55years old for a male and 65 years old for a female) is definitely an accepted risk factor, but not stress.
