I received the letter from Fidelity, which included a "reference file number". It stated "based on the communication received from the Issuer, the security in your account identified above is required to be held directly with the Issuer's transfer agent".
I called Fidelity to get more specific information and they said that I would receive a letter (in the next one or two months) explaining who the transfer agent is and how to contact them.
I told the Fidelity agent that there is quite a number of us who are extremely upset about this mess and we may consider legal action.
Further, my neighbors held Neah stock (less than 2 million) in their portfolios and received checks in the mail. My wife had Neah in her Fidelity account (also considerably less than 2 million) and her shares simply disappeared in December. No explanation - no check.
