Agree, a large number of manuscripts aren't published for many months and most of my published papers fall into this category.
Top medical Journals such as NEJM or Lancet are very interested in maintaining and improving their Impact Factor and the higher the number of citations a paper receives, the better for the Impact Factor.
For groundbreaking studies showing game-changing results, the Editors will work with the academic researchers or clinicians to fast track. I strongly suspect that the DCVax-L phase 3 data will be in this category.
