Remote Monitoring Program Manager

Job Overview: Oversee clinical monitoring program. Provide clinical and organization oversight to the Patient Monitoring program

Essential Functions

1. Plan, develop, organize, implement, and lead Chronic Care Remote Patient Monitoring Program

2. Monitoring Clinician Portal for our client medical practices and encouraging patient participation in their home monitoring program. This will be achieved via messaging within the app and live phone calls and video chats with patients

3. Assesses and evaluates patient data and needs, providing appropriate support.

4. Notifies physician and other care agencies of changes in patient condition or status, sends reports, obtains new orders, and modifies the care plan as needed

5. Maintains clinical skills within nursing practice norms and participates in group projects, training, and education as needed

6. Informs appropriate personnel of perceived problem areas related to program, patient care

7. Demonstrates a professional attitude, sound judgment and continuity in interactions with physicians, CareClix staff, and other health care providers

8. Develops and promotes cooperative working relationships with other departments, hospitals and community agencies.

Job Requirements

1. Preferred Education: Bachelor's degree in nursing

2. Appropriate certifications and/or licensures with 2-5 years of experience; or equivalent combination of education and experience

Supervisory Responsibility
This position will oversee the clinical staff responsible for monitoring remote patient data.

This is a full-time position. This position regularly requires long hours and frequent weekend work.

Travel
Some out-of-the-area and overnight travel is required.
