What do you mean "ICER response to MRC"? None of the stuff you posted is in the ICER doc - where did you see these responses, and secondly, why would ICER even respond to a CP that was filed with the FDA? For instance, the term "Bhatt" can only be found in the reference list at the end of the report, plus under two tables related to efficacy that refer to the two papers he's published on R-IT results.

I believe it was in reference to p. 12 of "ICER’s response to public comments received on the draft evidence report."
