Yes. That is the more precise way to model it.

A patient can have more than one MACE event. But he can only have one FIRST MACE event.

If a patient has a non-fatal MI and then a year later has a second fatal MI, both are events that are adjudicated but only the non-fatal MI will be included in the primary endpoint. The later fatal MI will be included in one other secondary endpoint; "total mortality".

Take the AIM-HIGH trial for example. There were a total of 72 deaths from coronary heart disease, but only 46 of them were first MACE occurrences.
