Does UHC use Express Scripts as their PBM? If so, I would think the PBM's rules over-rule the insurer's rules - that's what they're there for, to lower drug costs, to save money for their clients - not to make patients healthier.

I think you're saying that you can't get V covered regardless of TG levels, correct?
