We probably won't because the average investor doesn't understand the significance. I think it's safe to assume that the board members have seen the SPORT and were impressed enough to join Dr. Fowler. They are all at the the forefront of MIS. The average investor can't connect the dots and wants FDA approval and sales IMO. We should just keep buying more shares and be patient.

I got a bigger nugget! Dr. Lee was recently appointed the Director of the American Board of Surgery. He is the the highest ranking surgeon in academia. The ABS writes the exams for all surgeon to stay board certified. BIG DEAL to me.

What different opinions? I saw the post this morning before church and I was disgusted. I wanted to email administration this afternoon but I saw it was deleted. Good. On a positive note check out this post from the American College of Surgeons Message board. It was written by a much older surgeon than myself which is refreshing. Maybe 60 something. I can totally relate to what he is saying. A lot of hernias need repair. SPORT will rule IMO;
TEP, TAPP, Robotic, open, plug and patch, to tack or not to tack, what tacks, etc. If there is an afterlife then Bassini and Halstead are rolling over in their graves right now. As an older surgeon who spent most of his life doing the old fashion operations mostly the Lichtenstein procedure (really a tension free version of the Bassini with mesh) I have joined the dark side (perhaps the darkest) and am now very happy with the robotic TAPP. The cost argument is a moot point. The Da Vinci has metastasized to our institution a decade ago, so no actual cost there. The trocars are reusable, again no cost. The instruments have a 10 uses life something around $600 per case. I use 3-0 Vicryl to tack in the mesh in two locations and close the peritoneum ($2.50) and of course the mesh ($100) . Having survived (successfully) one inferior epigastric artery hemorrhage law suit, blind TEP seems dangerous to me at least in my hands. The balloon is better but adds $600 to the cost. The advantage for me using the robot is the 3-D magnified vision, the ease of intra-corporeal dissection and suturing, and the comfort of sitting (getting more important all the time). The dissection of the groin from the other side is really quite elegant with the robot as compared with the blind TEP. One can really appreciate the structures such as the transversalis fascia that were marginally identifiable and required a bit of imagination with the open approach. I have also frequently enough noted that whatever really causes hernias has a bilaterality to it with a companion hernia on the other side that I could not feel on exam, something that would be an unknown to the open or TEP surgeon. I will add one more issue, the PR and sex appeal of the robot doesn't hurt matters either.
Interesting history. One of the worst deals ever! Also, Dr. Fredric Moll (the founder of ISRG) used to work for Guidant and was unsuccessful in convincing the company to purchase the robotic technology used for the initial da Vinci. They thought it was to "futuristic". He later left Guidant and started Intuitive Surgical.

From what was explained to me by Dr. Fowler is that a design freeze is a point of no return. That's the design that gets submitted to the FDA. Any change after that means the FDA process would have to start all over.
