This is getting to the heart of locoregional therapy (tace, ablation in hcc) versus systemic treatment. Many of these approaches have been shown to be of minimal benefit when treating one or several of the multiple metastases. There are always more unfortunately. Therefore the assumption is that even if you could inject all of the ones you see (resolution on CT is probably 1cm masses), the smaller ones that are clinically and radiologically occult would still thrive. The immune response is absolutely essential for this treatment to be truly worthwhile.
