JL , Zoo ,Ajax and others would be more qualified then I to comment on Epanoza
The Cardiologists I know would be concerned about the increase in LDL-C. .....not as bad as Lovaza but still an increase
I wonder what attracts AZN to the deal ...the only thing that comes to mind is to be able to offer a combo pill to compete with a generic Lipitor / Vascepa combo .
AZN's Crestor is the strongest statin .....lowers LDL -C on average dose about 10% more then Lipitor.
So as a combo pill with Epanoza they could try and make the claim it's the same overall benefit as a generic Lipitor combined with Vascepa
The Cardiologists I know seem favorable to the idea of combo pills to both lower cost and increase compliance ......and are still very stuck on statins as the first line of drug action ( after diet , exercise and no smoking )

Ah yes Epanova's great claim to fame...It lowers ApoC3..Now why didn't Lovaza think of that...

Re some of Epanova's claim...Naturally occurring fish oil EPAs and DHAs come in in triglyceride form...They are broken down in the small bowel into individual strands and reconverted into trig form in the blood stream...converting fish oil components into ester form is done because it is difficult and expensive to produce concentrated EPAs and DHAs using the unmodified forms. There is some literature that suggest natural form is more bioavailable, but this is pretty sketchy and the blood and RBC EPA levels for Vascepa are fine by FDA standards...

ApoC3 is a lipoprotein found in the VLDL where most trigs are transported..It's only present in quantity if your trigs are very, very high or in you are very obese..It might be a risk factor in those cases because it interferes with lipoprotein lipase near the endothelial lining...It is nowhere near as important as ApoB which is the major risk lipoprotein.

DHA might be a more potent drug if lowering the trigs is your only concern...Not surprising it might lower ApoC3 more than EPA, but in most cases this is really not very important...

Face it..if this stuff was great, they wouldn't have sold the company for $325mil.
