If you're referring to etrade. I just got off the phone with corp actions. They are saying now to YES, fax OR MAIL pages 6, 9, and 10.

The process is:

Create a cover sheet and address it to

Attn: Corporate Actions Department

Add acct# to your cover page

The fax number is [phone]

Sorry I gave etrade some props for making it painless last week. This is a PITA since I now I have to walk family members through this BS process... That rep on the phone said they were sending out alerts to everyone that voted before they got their crap together. I say BS since I do not see any alerts on my etrade or email.
